Misconduct Reports
Referees are required to submit a Referee Report (formerly known as the “Referee Misconduct Report”) for any Send-Offs of players, team officials or spectators that occur in Area 1D play within 48 hours of the game. This same report is also now used for the referees to report any Serious Injuries which occurred during a game.
NOTE: Cautions issued by the Referee are to be reported on the back of the lineup cards; a separate Referee Misconduct Report is not required.
Incident Report Form
An AYSO Incident Report Form is used whenever there is a personal injury, damaged property, or threats of or actual physical violence surrounding an AYSO game, practice, event or property. The form should be prepared by the coach, AYSO Official, or AYSO Volunteer or by tournament or event staff members. Instructions are included within the linked document. For all Area 1D 16U/19U matches and 10U/12U/14U Playoff or Spring League matches, you should forward your report to the Area Director within 48 hours.
AYSO Referee Feedback Form
The Referee Feedback Form is provided so that you might help our volunteer referees improve their knowledge and skills. Please feel free to offer your comments, be they praise and/or suggestions for improvement. Your submission will be forward to the Area Referee Administrator for review.
Application for Referee Certification Form
AYSO/USSF Cross-Certification
Cross-Certification to USSF is available to AYSO Intermediate, Advanced or National referees who have been certified as referees for six months or more and who are not currently certified with USSF. All games officiated for AYSO or USSF may be applied toward the certification requirements of either AYSO or USSF.
